I've been making these for a while now, but I'm surprised by how many people don't know about them, since the design is elegant and simple.

There are a number of articles on the web that explain how, and even generators to help you arrange the information on the paper, but this article from the Athens Musician Network gives you a good explanation and an excellent set of photo instructions.
